FEES

• Your payment or co-payment is due at the time of service for all visits.  We accept cash, check, Master Card and Visa.  We do not accept American Express or Discover. If necessary, other payment options are available by working with our billing office manager. However, you, as the patient or responsible party are ultimately responsible for the account balance.

INSURANCE

• We are contracted with most major insurance carriers. If you have a question concerning your carrier please contact our office.  We file all insurance claims directly from this office and process all necessary information required by your carrier.
  Please bring your insurance card with you so that your claim can be filed with the correct carrier.
  
  
• Some insurance carriers require a referral authorization number for your visits as well as precertification for other scheduled tests, x-rays, and surgeries that you may need. Please obtain the authorization as well as the referral prior to your scheduled visit.
